Gout and Lifestyle—Debunking Myths in the West African Context
Gout is frequently dismissed as a "disease of kings" or a simple consequence of overindulgence, but for thousands of Ghanaians, it is a debilitating chronic condition that requires serious medical management. As urbanization increases and dietary habits shift across West Africa, the Rheumatology Society of Ghana has observed a sharp rise in gout cases, making it one of the most common reasons for rheumatology consultations in our clinics.
A common myth is that gout is merely a temporary flare-up caused by one "bad meal." In reality, gout is a form of inflammatory arthritis caused by an accumulation of urate crystals in the joints. Without proper long-term management, frequent attacks can lead to permanent joint destruction and "tophi"—visible lumps under the skin that can become infected or ulcerated. Furthermore, untreated gout is often linked to other serious health issues, including hypertension, kidney disease, and cardiovascular problems. It is a systemic issue that impacts the entire body, not just the "angry" joint that hurts during a flare.
In Ghana, many patients attempt to manage joint swelling with over-the-counter painkillers or herbal remedies. While these may provide temporary relief, they do not address the underlying high levels of uric acid in the blood. Our goal at the RSG is to promote a "Treat-to-Target" approach, where medication is used not just to stop the pain of a flare, but to lower uric acid levels permanently, preventing future attacks altogether. We must move away from reactive medicine and toward proactive, lifelong management to prevent the long-term disability that chronic gout can cause.
Dietary modifications—such as reducing the intake of organ meats, sugary sodas, and certain alcohols—are important, but for most patients, they are only one piece of the puzzle. Genetics and kidney function play a much larger role than most people realize. It is unfair to place the entire burden of the disease on a patient's lifestyle choices when their body simply cannot process uric acid efficiently.
